As of now nothing has been submitted and you’ll be following up with the guardian Jessica Lopez Thursday who is assisting you with gathering the documentation. The custodial authorization is set to expire May 23,2016 and will not be extended till UHC receives confirmation the 2159i and conversion package was submitted. Please continue to keep me updated.
