I have enclosed three fitness evaluation report enclosure (1) is for the period prior to my GOVCC misuse, demonstrating good judgment, trustworthiness and reliability. Enclosure (2) is the adverse fitness report for the period I received a 6105, also acknowledging my negative behavior, and accepting the consequences of my behavior. Enclosure (3) is for the period after my treatment, and recovery, showing renewed good judgment, trustworthiness and reliability. During this period 2015 I was awarded a Certificate of Commendation signed by Brigadier General Michael A. Minihan USAF, Colonel Christopher J. Mitigating Circumstance. I was misusing my GTCC for personal reasons due to my gambling addiction I have enclosed residential treatment completion with a diagnosis of Gambling Disorder Severe enclosure (5) I have received, counseling for my disorder and my current condition is being resolved and under control. e. It was discovered through a command investigation that you misused your position as the Command's Agency Program Coordinator (APC) and Approving Official (AO) to increase your cash withdrawal limits from your GTCC. It also revealed that you placed your card in "mission critical" status to avoid paying the monthly bill. All of which was to cover up your transactions in order to support your gambling habit. f. Mitigating Circumstance. In my attempt to demonstrate an initiated good-faith effort to pay my GOVCC even though I was misusing my position as an APC. I have included the GOVCC statements showing monthly payments made during the period of my misuse enclosure (6). The intent behind placing my card on mission critical was, in the case I could not pay the GOVCC for that month. The mission critical status would shield me from showing up in the delinquency list. While acting as APC you conducted 52 maintenance transactions including activating and extending the activation, increasing credit and withdrawal limits and assigning GTCC' to a mission critical status. h. As the acting APC I had the ability to conduct my on maintenance transactions to include the activation and extensions, increasing my credit and cash withdrawals limit and assigning my GTCC to a mission critical status.
