This letter is to request reimbursement for medical expenses. We were visiting Amman/Jordan on 14 Jul 2016 for three weeks. The second day we arrived my son Mohamed Al-Asfar had a sudden severe pain in his tail bone along with Pus and blood draining out of it. Mohamed had a Pilonidal cyst, Consequently, a pilonidal sinus excision was done for him at The Islamic Hospital in Amman -Jordan. The amount of 990 Canadian dollars (542 Jordanian dinar) was paid by me.

We arrived back to Canada on Aug 7 2016. Accordingly, our family physician Dr. Gharagozloo, Mehrdad examined the wound and referred Mohamed to CCAC for dressing every other day.
Please find enclosed all the documents related to my son Mohamed Al-Asfar surgery.
