The assessment tool SIGECAPS was used to access for depression. Although the patient had been previously diagnosed with major depressive symptom with a single episode, there was the suspicion of possible recurrent major depression with psychotic features. She admitted to difficulty falling and staying asleep, decreased interest in activities especially in school, somewhat guilty feeling about her parents getting a divorce, decreased energy, poor concentration, and decreased appetite. She denied any recent weight loss or weight gain. She denied any suicidal or homicidal thoughts during the interview. The patient was not very open with her response to questions, she continued to look at her father signaling him not to say certain things.
When interviewer tried to ask more questions about one of her imaginary friends called Chris, she seemed agitated by the question. She only voiced that Chris wanted them to do something on the bathroom floor, but refused to explain further. Hence, the prescriber was not able to assess further on the patients psychotic features. Her parents want her to get a job and it scares her thinking about it because she is worried about getting along with other people. She denied any panic attacks or feelings similar to it. She denies any reoccurring headaches or stomach aches. She reported having a difficult time relaxing sometimes because she feels like she has to worry about everything. She occasionally worries about something bad happening to her or members of her family as she used to be bullied and teased a lot in school. Symptoms are congruent with DSM-V (2013) requirement for an anxiety disorder. She was previously diagnosed with generalized anxiety and still meets the criteria for it at this
