General Appearance and behavior: The client’s name was Alan, he appeared calm and open to counseling. At times, the client would lead forward to speak and appeared to be relaxed while discussing his issues. His behavior was normal and consistent as the client explained his problem. The client used hand motions at times to explain his story. Presenting problem: The presenting problem is that the client has an uncontrollable gambling addiction that has caused major problems in his personal and work relationships.
History of the current problem and related problems: The client stated that they had won a total of 2.8 million dollars in the Powerball lottery at age 26. The client quit his job and opened his own construction company. The client’s

The client is having issues with his wife that leads to major arguments because of his gambling addiction. The client is constantly absent from work and takes money from the work fund to gambling. His family and co-workers have noticed an extreme change in function and behavior in the client. The client states that his relationship with his wife is out-of-control because of his gambling addiction. The client has a lack of leisure activities that he once enjoyed. The client stated that he enjoyed reading and playing video games; however, his current and past jobs had taken up from his free time to enjoy these activities. The client states that he once would read a book at night; however, his late night poker tournaments have taken up his time that was once used for reading or other leisure activities.
Use of alcohol or other drugs, including medications: The client mentioned that he was only a social drinker. He stated he only drank occasionally with a few friends or family and did not consider himself a heavy drinker. The client stated that he did not use any other drugs or medication now. Client stated that the reason he has not used drugs was because friends in high school used drugs and he did not want to turn out like his

However, a suicide assessment was still conducted to assure if there were any risk factors. The assessment concluded that the client was not at risk to harm self or others.
Previous counseling: The client mentioned that he had attended a Gamblers Anonymous meeting; however, he did not complete the meeting because he did not find it useful. Apart from the GA meetings, the client had previous attended counseling. The client stated that when he was ten years old he attended counseling after the death of his uncle. Counseling was needed because his uncle had raised him since his father was not present. The client’s uncle passing away caused great devastation to the client that counseling was needed for the client.
Attitude of client toward the counseling process: The client seemed optimistic that counseling might solve his current gambling addiction. The client was open to counseling and cooperated when answering the questions asked. The client answered the questions with interest and in details.
