6/19 6pm Worker went to the of fran kortkamp for a home visit. When the worker arrived at the home fran came to the door and appeared as if she has just woke up from a nap. Worker sat down at the kitchen table and spoke with fran. Fran stated that the children both have a cough. Fran stated that she has an appointment set up with a new psychologist, Jackie hall Davis, in October. It was 250 for the initial visit and 100 each time after. Worker spoke about getting the psychological evaluation appointment set up. The evaluation had to be done by a DcFs. Fran spoke about Zoey attending camp in July and august. Worker spoke with fran about to testing. Fran states that she believes the doctor should have the TB testing on file. Fran states that Zoey has to stay in outpatient until she proves that she is …show more content…
Fran states that summer school is not going well. Yaw called and said that there needs to be an agreement because Zoey will not cooperate. Yaw states that if Zoey would just complete her work she could be finished with summer school. Fran states it was it is because Zoey overheard the worker speaking to ms franke talking about how Zoey did not have to attend summer school because her grades would not be affected. Fran states that she is having difficulties with bus transport to summer school. The children had seen their parents while staying with Kristi. Zoey stated they had cake and pizza. Chris had bought the cake and pizza. Fran informed the worker that at the visit with sultana nothing happened. Sultana states that she is the attending psychologist. Fran is concerned that Zoey is on too much medication because she is not sleeping. Sultana suggested that Zoey take melatonin. Medication is making Zoey act different. Work spoke about birth control being set up by the primary doctor. Fran made a deal with Zoey if she behaves she could go to the zoo with daycare. Worker spoke about the children needing their finger prints. Fran states she still has the
