In the article JCAHO “Forbidden” Abbreviations, I learned how easily someone can get confused with an abbreviation and think it is something else. That is very dangerous. If someone uses D/C for discharge, another might interpret it as “discontinue”, and that could be very threatening because they could discontinue a patients medicines that they might need.
