Objectives:
• Review previous meeting action plans and discuss any questions/issues pertaining last week follow-up medical record data entries.
• Identify and complete this week follow-up medical record data entries.
• In order to improve data capturing, will review medical records of subjects who has an appointment with lipid clinic within 30 days of follow-up window period, and accordingly will plan follow-up medical record checks.
