Follow Up Inspection

An unannounced follow up inspection was conducted on this day. I arrived at the operation which is located at 602 W. 12th St, Texarkana about 9:10 am. I was greeted by Nancy Parker and Linda Offield the directors. The purpose of the inspection and the subchapters that would be evaluated were explained to Nancy Parker and Linda Offield.
Preparation for the inspection included reviewing frequency of inspections made, observing the operations compliance history, the operations fee history, background check list and controlling person list, etc
The Director was present. Nancy Parker qualifies under 746.1501(a)(7) which is non-expiring license and Linda Offield qualifies under 746.1015(a)(3) which is non expiring license. Director Training was

These vans do not require safety alarms as they were purchased before 12/31/2013.The vans are used for Field Trips and to/From Public Schools.The center utilizes a Transportation Check List/Rollsheet and uses name/face checklist as a secondary system.Computer generated document is used that tracks the dates and expirations for expiring training and certifications such as transportation training, CPR/First Aid, and drivers licenses. Ride along employees are required to have CPR/1st Aid and Transportation Training. I observed at inspection the caregivers relaying to the caregiver in the classroom how many children got off the bus and before the van driver left the room the caregiver counted and did a face to roll check. The van driver than did another sweep of van and parked the van in the designed spot.
Field Trips: They are only conducted during the summer only except for two field trips for the Pre-K in the fall to fair and pumpkin patch. In the summer each child is given a calendar of field trips for summer and then they post a note on the front door. The director is always present on the field trips and an additional 2 caregivers even if the ratio is met with one caregiver. The caregiver completes transition logs that are check throughout the field trip. They also conducted a van log when enter and exiting the vans. Pools: not on
