His primary condition is a T1 spinal cord injury due to a removal of a tumor. His secondary conditions include unspecified injuries between T2 and T6. The client suffers from muscle weakness, LE weakness, has difficult with transfers, and complains of constant pain. Between OT and PT the client has had 94 therapy sessions. Some performance skills prominently affected are his motor skills for example when he bends, reaches, and moves. Some occupations that are affected by these performance skill deficits include, functional mobility, toileting, dressing, and meal clean up are naming a few. The clients current level of performance in self-care, work, leisure, and play is suggested to be stand by assist (SBA). Stuart’s psychosocial impact is very positive; he understands his disability better than anyone taking care of him. He knows his dos and don’ts, he is optimistic to go to therapy regardless of feeling a lot of pain, he accepts his mistakes, and he takes care of himself. He described during therapy about getting into an accident over the weekend, and stated that he shouldn’t have been driving as he was very tired.
