(2013) started introducing the potential practice for the organization to implement by determining an area for improvement based on evidence. In an effort of improving patient outcomes and decreasing the health care costs (Baptiste et al., 2013), proposed the utilization of The Self-Care HF Index (SCHFI) tool. The SCHFI comprised of 22 questions which pertained to patients’ self-care abilities, in the areas of “self-care maintenance, self-care management, and self-care confidence” (Baptiste, 2013, p.54). Titler (2008) supported that EBP recommendations should be supported with the type and strength of evidence used to determine if the evidence findings are appropriate for implementing its use in practice. She further added that evaluation of the EBP changes, staff education and support all play a critical role in the successful implementation of EBP (Titler, 2008). In this study, however, approval from the ethics committee was not factored, which the writer considered a flaw in conducting the study.
Ethics protect patients’ privacy and anonymities. An informed consent would warrant a formal participation in the study and still highly recommended.

(2013) concluded that patient education focused on self-care management have a significant impact on decreasing readmissions and improving the quality of life of heart failure patients. The use of SCHFI instrument to measure self-care abilities has proven to be useful after hospitalization. Self-care management and practices promote independence and control, which can increase patient satisfaction. As nurses, we continue to pursue our goal in providing the best care to our patients, and this study by Baptiste et al. suggested that nurses continue to play a significant role in the promotion of health and wellness, as well as decreasing health care
