In the future, I hope to become a hospital social worker, and discharging people will be a huge aspect of the job. When a resident gets discharged, they get asked a series of questions. The first set of questions has to do with cognitive functioning. For example, they will get told three words, then a little while later they will be asked to recall those three words. This is easier for some residents versus others. In this section, they also get asked what year, month, and day of the week it is. Some residents can answer these questions correctly, while others still believe the year is 1967, it all depends on the residents cognitive functioning. Next, comes the mood section of the questionnaire. These questions are self-explanatory, “do you get pleasure in doing activities, do you feel depressed, are you fatigued and overly tired lately.” Furthermore, after all the question have been answered they then are documented into the computer. After this process care plans can be made to promote a better well-being for the resident.
