My field placement at the Western Homes has officially came to an end. During my time at Nation and Thuesen Cottage I have learned an incredible amount of information about gerontology social work. At my field placement, I had many responsibilities. Mainly I visited with the residents and gave them a form of socialization and built relationships with them. While I was there, I also helped my supervisors with various tasks. These tasks included helping set up for various events, helping with the discharge and admit process, and observing all the social work aspects of gerontology. During my time at the Western Homes Cottages, I learned many valuable things. One of the many things that I have learned is how to promote a better well-being for …show more content…
In the future, I hope to become a hospital social worker, and discharging people will be a huge aspect of the job. When a resident gets discharged, they get asked a series of questions. The first set of questions has to do with cognitive functioning. For example, they will get told three words, then a little while later they will be asked to recall those three words. This is easier for some residents versus others. In this section, they also get asked what year, month, and day of the week it is. Some residents can answer these questions correctly, while others still believe the year is 1967, it all depends on the residents cognitive functioning. Next, comes the mood section of the questionnaire. These questions are self-explanatory, “do you get pleasure in doing activities, do you feel depressed, are you fatigued and overly tired lately.” Furthermore, after all the question have been answered they then are documented into the computer. After this process care plans can be made to promote a better well-being for the resident. Although this process may be different from a hospital social work stand point, I still found it a very helpful process to

Moreover, hospitals are now calling patients at home after being discharged to ensure that they understand the discharge planning and instructions and to answer any questions they might have or concerns regarding their discharge. They are also calling to ensure that they understand the medication regiment and all the instructions that come with it. It is also important to understand that hospitals are ensuring patients are being followed up with home care nurses and sometimes physicians to ensure that they are following all the discharge instructions and to take care of any medical needs they might have such as dressing change, taking their vitals and ensure that they are following their discharge instructions. The hospitals are doing this so as to prevent them from being penalized for patients returning to the hospital for readmission after 30 days (ACEP, 2015). It is important to know that as a result of the penalties that hospitals are faced with, they have now implemented a plan or policy whereby to allow physicians to be made aware of their patients being discharged, and to follow up with patients on lab work and diagnostic testing and to ensure…

In addition to working with the Aftercare groups with Mary and Leslie at the YMCA, I believe that working directly with Jami Geist, who is the oversees the residential units to be of the utmost value. Collaboratively, Jami and I pull files of clients from a list of discharges, and I call these people, providing they gave consent, and ask various questions to gage whether we are having success with helping people become more self-sufficient, and live healthier lives. This has been an important exercise for me to witness what works, and evaluate what is not, and listen to important details in what people…

Skilled therapy is then carried out multiple times a week instead of the Walk-to-Dine Program for the resident. A therapist will work on the areas that the resident showed a functional decline in and promote physical ability and safety awareness where possible. Upon discharge from skilled therapy, the resident should be placed back on the Walk-to-Dine program. The Walk-to-Dine program will then be carried out again by either restorative nurse or the CNAs. The program does require a little more time management with each resident to achieve the programs goal.…

As an occupational therapy graduate student, I had already decided, working with seniors was not for me. I wanted to work with exciting clients, in an exhilarating envrioment; not in a nursing facility. However, as I had just learned, I would I would be stuck there for the next six weeks observing and assisting an occupational therapist. On my first day, I slowly pulled into the parking lot. “Well here I am,” I thought as I got out of the car, “it’s only six weeks, I can do this.”…

Volunteering at Wisconsin Lutheran Care in Wisconsin I had the opportunity to observe geriatric care. While the facilities were well maintained and the technology at that time seemed appropriate, the treatment that these individuals received was subpar. The clinicians showed little eagerness to go above and beyond and treated the patients as if every one of them was the same. The patients received no personalization in their care and therefore, I felt that they were not receiving the type of commitment that I would expect from the clinicians. There was clearly a lack of care for this underserved community.…
