I, Kylie Medlin, a second semester Junior at Campbell University, propose that my 200 hour supervised field experience in an off-campus setting that is appropriate to my course of study, be completed at Cape Fear Valley Rehabilitation Center under Rhonda Moore-Sweitzer OTR/L. It is my goal to become an Occupational Therapist by attending a Master’s Program upon graduation. I will begin the application process to the Occupational Therapy schools starting July 1st, when the first application becomes available. The three programs that I am applying to are East Carolina University, Lenoir-Rhyne University, and The Medical University of South Carolina. This internship will provide opportunity to learn more about the field of Occupational Therapy, allow me to practice applying what I have been learning in the classroom, and will allow me to further my love for this field by watching patients return to their prior level of function and normal activities of daily life because of undergoing Occupational Therapy. …show more content…
This center is located on 1638 Owen Drive, Fayetteville North Carolina. The contact number for the Rehabilitation Center is 910-615-6948. My supervisor, Rhonda Moore-Sweitzer, OTR/L is a graduate of East Carolina University’s School of Occupational Therapy, and is the head of the Adult Outpatient Neurology unit. Her best source of contact is by email at, rsweitzer@capefearvalley.com. I have been given permission by Mrs. Rhonda Moore-Sweitzer to observe her every Tuesday and Thursday from 8:00am-4:00pm. This supervised field experience will commence on Tuesday, January the 17th, and will continue no later than April the 27th, Campbell Universities Reading Day, or until the required 200 hours have been
