On 01/27/2016 Ms. Vasquez came to the SSO for an ILP meeting with CM Roman. HS De La Torre asked client to stay in order to develop a Housing Plan to explore housing opportunities available for the family. Client agreed to stay and the following was discussed during the meeting with HS.

Ms. Vasquez informed HS that last permanent address was located at 674 East 232nd St. Bronx, NY for one year (2014-2015). Family was evicted from the apartment. Family is currently residing at 1195 Sherman Ave. Bronx, NY Unit 32A, admission date 04/18/2015. Family composition consists of Virginia Vasquez (self, 39y); Skye Weeks (daughter, 3y); Jordan Weeks (son, 7y); Breonna Weeks (daughter, 14y). Ms. Vasquez stated if permanent residency is obtaining no one else will be moving with the family. Further, Ms. Vasquez stated that she is not pregnant either been convicted of a crime or been on parole.

Ms. Vasquez is employed at the moment and earns about $900 bi-week. As per Ms. Vasquez, she is saving money however client has failed to provide proof of savings. Client PA

Voucher will expire on 02/07/2016. HS provide an updated broker lists, housing search log and apartment review checklist. HS reminded client that family must view 2 to 3 apartments on weekly basis and provided proof of viewing. Ms. Vasquez stated that she is not able to seek intensely because, she is on probation at her employment due to lateness and she is on the risk of losing her job. Client stated that she does not have time to look for apartments at the moment. Client will be referring to Bronx works for a screening for the CITYFEPS program. Furthermore, HS explained Ms. Vasquez about the other programs, EOSD, COME HOME NYC, Section 8 and Project Reunification, Ms. Vasquez stated that she is not interesting on any of the programs.
