On 03/28/2016 client came to the SSO to meet with Housing Specialist De La Torre to develop a Housing Plan to explore other housing opportunities available for the family. The following was discussed during meeting:
Ms. Rodriguez informed HS De La Torre that last permanent address was located at 11811 Pricly Pear Seffner Fl. 33584 from 06/30/2015 to 01/25/2016, the family moved from this location because the apartment was overcrowded. Family is currently residing at 1195 Sherman Ave. Bronx, NY Unit 32B admission date was on ¬¬¬02/07/2016. Family composition consists of MARIA RODRIGUEZ (HoH, 31y); YELENNY APONTE (child, 12y); DIANEL LUNARODRIGUEZ (child, 12y); JOSUE SANTOS (child, 6y).
