Circumstance: Ms. Smalls (MHP) made monthly contact with Deonte’s family and Deonte.
Action: MHP conducted monthly visit with Deonte’s family and Deonte. Alex and Ryan (MHS) reported Deonte is doing well in school. Deonte continues to have some issues with peers and talking during teaching time. MHP provide MHS with SAFY updates and information needed on upcoming training. MHP and Deonte discuss added personal belongings to his bedroom and school updates. MHP asks Deonte about his behaviors and treatment in the home. MHP and MHS discuss the last court date being cancel and possible outcomes. MHP schedule next schedule visit with MHS.
Plan: MHP will provide Deonte and family support, and continue to ensure Deonte’s well-being and safety.
