Pts is an African American woman that came in with a GSW in her UE elbow. She had reconstruction surgery on her whole elbow. (S) Subjective
She answered the therapist questions about her pain level being a 6. Also made remarks about her elbow giving her problems in her day to day task. Pt said she was doing her home exercises. (O) Objective
Pt slouched in her chair, and had barley any movement in her elbow during activities or passive range of motion. She did not talk that much unless she was asked a question by the therapist. (A) Assessment
