At today’s she is found sitting in her room. She is awake, alert and oriented times1. I am here to follow up per the facility nurse request for the patient’s pain and functional decline. The patient reports chronic, dull, lower back pain, severity 3/10. She states that her pain regimen is helping her pain because without the pain medication she has “great back pain”. The facility staff reports that the patient has chronic itching; she picks at her skin and creates wounds. She current has wound to BLE. The patient denies shortness of breath and chest pain. She reports fair appetite and having regular bowel movements.
