Scenario
For confidential issues, the patient will be called ‘John’ henceforth. John is 4 years old who was admitted into the hospital for orchidopexy. He has bilateral undescended testicles. He was nil by mouth from 06/10 and he came in with his mother. John was under my care primarily, with my mentor supervising and supporting. I will focus on John’s post operation scenario.

(NICE, 2007, Citied in Nottingham University Hospital, 2011) His observations on admission were; He was alert, active, well perfused and looked well. Heart rate – 93, Respiration Rate – 20, Blood Pressure – 102/49, Saturation – 99%, Capillary Refill Time (CRT) – 2 sec and Temperature – 36.2 and his Paediatric Early Warning Score (PEWS) was 0.
The PEWS is essential in providing effective care as it supports the nurses in making assessment and recognising early signs of deterioration as well as providing guidance on the level and frequency of monitoring required in preventing further deterioration. Post operatively, on arrival to the ward his observations were satisfactory, he went to sleep with no complaints of pain. I kept John on regular observation due to his operation being under general anaesthetics. His PEWS score remaining 0 throughout the observation and his observations remained stable throughout. His wound site was clean and no signs of bleeding or discharge of exudate, he seemed a bit sore around the area on touch however the area was blanching with CRT of <2.
