• What factors contributed to this type of error?

Several factors contributed to this error, first the student did not check the labels before given them to the patient. If she had checked the labels she would have notice that the medications were the wrong ones. She did not check the right medication or the right reason. Secondly she did not identify the patient using at least two identifiers.
• Should the nursing student fail the course? Why or Why Not?
I don't think the student should fail the course if she reports her error to the clinical instructor. If she reports her error the student is behaving in an ethical manner and is safeguarding the patient well being. If she decided to hide her mistakes and the instructor finds out
