Exercise Program Summary

An AEP places emphasis on implementing achievable and sustainable lifestyle modification strategies aimed to empower an individual towards greater independence and self-management of personal health and wellbeing (Lederman, 2016). Clients place many barriers which need to be broken down throughout a program especially concerning chronic illness and mental health conditions to achieve significant positive health changes (Morgan et al, 2013). All aspects of Mrs. G. Patterson’s program will be reviewed, regressed or progressed as required at each session. Her program timeframe has been developed to enable changes to be implemented:
The Initial Assessment focuses on education of hypertension, OA, weight loss, planning exercise schedule in dairy and investigating local group exercise programs. Also, introduced a home-based resistant exercise program to start 2 times a week for 20 – 30 mins, 20 minutes’ aerobic exercise and stretching daily (minimum 3 times a week).

As the duration, intensity and exercise progresses through sessions, alternative activities may need to introduce to keep client motivated and interested in continuing program. Local group classes and gym sessions should increase to encourage social interaction and improving client’s wellbeing. A key component of the exercise program is education and exercise counselling. Breathing techniques used in Mrs. Glenys Patterson’s group yoga classes can be transferred to everyday life and positive achievements of her small goals will assist in building self-confidence. All exercise and learning activities will be a direct reflection of her identified learning style. Progress reports and regular contact with her other health practitioners will continue throughout the length of
