I spoke to Valerie Bomar, who stated one of her residents reported a sexual assault on 01\06\2016. Valerie stated Doris Leach reported a male nurse had raped her. Valerie stated the only male nurse working on 01\06\2016 was Dale Watiers. Dale was suspended for investigation. Valerie stated Doris was checked for any signs of trauma or abuse. Valerie stated there was no signs of any trauma to Doris or of an assault.

Valerie talked to Dale, who stated he was in Doris's room on 01\06\2016 to help her get ready for bed. Dale was helping Doris at the request of another nurse Raven. Dale helped Doris change into a gown and noticed she went to
