Evidence-Based Quality Improvement Project: Can Empowered Nurses Decrease CAUTI Rates?

I am writing to obtain permission to conduct an evidence-based quality improvement project using a pretest/posttest design in your facility with the purpose of evaluating the effectiveness of an educational intervention regarding the importance and use of the nurse driven protocol on nurses ' knowledge and CAUTI rates.
This study will be done to fullfill requirements for completion of the Doctor of Nursing Practice degree at Regis University, Denver, CO. The following information is an overview of the project: Can Empowered Nurses Decrease CAUTI rates?

Research Question: How will education on the importance and use of the nurse-driven indwelling urinary catheter removal protocol affect knowledge of nurses taking care of patients with urinary catheters and the CAUTI rates in an acute care hospital in North Carolina ?

Project Significance: An indwelling catheter removal protocol is a recommended tool and is part of a bundle used to prevent CAUTI. The lack of knowledge of its use and its importance may be directly related to high CAUTI rates
O Catheter Associated Urinary Tract Infections (CAUTI) can be reasonably prevented with the application of evidence-based prevention strategies (Institute for Healthcare Improvement, 2011, CDC, 2015)
O Each day the urinary catheter remains in place the risk of CAUTI increases 3-7 % per day (Institute for Health Care Improvement (IHI), 2011)
O Urethral catheters can lead to patient discomfort, morbidity, and even mortality (Saint, 2010)
O 41% of physcians and nurses fail to follow Center for Disease Control and
Prevention (CDC) recommendations to evaluate and determine if a catheter should be

Increase in knowledge will increase compliance in use of the nurse-driven protocol, and cause a subsequent decrease in catheter usage hence a drop in CAUTI rates. Studies indicate that the use of nurse-driven protocol to remove unnecessary catheter showed a significant decrease in CAUTI rates. (Mori, 2014;Center for Disease Control and Prevention (CDC), 2015; Institute for Health CareImprovement (IHI), 2011). In a study by an accountant (Scott, 2009), the annual cost for hospital due to CAUTI was between 340-370 million dollars. Therefore, a decrease in CAUTI rates could mean millions of dollars in savings annually for the hospital (Kennedy, et al., 2013). Example: The estimated hospital cost for one CAUTI is $1200-$4700 (Umscheid, et al., 2011). If the hospital reported 14 CAUTIs ,a decrease of even 2 CAUTIs over a three months period, would a potential estimated cost saving of 14,400-56,400 for the institutition.In addition, the hospital will regain its prestigious standing on Hospital Compare and may be saved from a potential decrease in Medicare reimbursement of 1%.
