The proposed solution in the evidence-based practice (EBP) project is the plan to overcome an identified problem. It should be reasonable and accomplishable within the time frame and the budget constraints. The evaluation of the outcomes should be quantifiable by the empirical evidence (Roberts & Yeager, 2004). The evaluation will enable one to determine the limitations in the implementation, its correction and to support the desired effects to get the best results (Melnyk, Fineout-Overholt, Stillwell, & Williamson, 2010).

The discharge nurse needs to instruct the patients and their family about the benefits of monitoring the daily BG levels and the possible complication if they fail to comply with it. The barriers could be the language barrier and the inability to comprehend the way to maintain the BG monitoring tool. By providing the instructions in the patient’s language and encouraging the patient to request to the staff who conducts the BG test to record and interpret the result to them, one can solve these problems. The primary assumption is that the BG monitoring chart would work as an effective reminder. It will make the BG level checks a daily routine and will help detect the inadequate glycemic control. Once the patient identifies the issue, they could seek medical attention and thereby avoid the risk of developing the SSI. The two limitations identified are the non-compliance of the patients in spite of all the adopted measures and the BG monitoring equipment
