Early Childhood Observation Summary

General Information: Emma is 5 year old girl currently attending Mrs. Engel’s early childhood afternoon classroom. A re-evaluation was requested in order to gather additional information in preparation for her transition into Kindergarten.

Emma currently received direct occupational therapy services. Per chart review Emma received early intervention services through the Indiana First Step program prior to attending the Early Childhood program.

Summary of Procedures: Emma was evaluated in small office at Protsman Elementary School along with observation within her classroom. The School Companion Sensory Profile 2, VMI and observations were used to assess her current strengths and weaknesses.

She attempted to skip which is an emerging skill.

Scissor Skills: Emma independently donned a pair of scissors. She cut out a triangle from an index card staying within ¼ inch of the reference line. She can cut out a circle but doesn’t turn the paper all the way and will segment her cutting.

OT Summary: Emma has lots of energy; however her high energy level results in difficulty following instructions, grabbing objects, difficulty focusing on tasks and difficulty completing tasks before wanting to move on to another task. Emma is able to be redirected with verbal reminders of the “rules” but needs that reminder repeated between steps of an activity. Emma’s strengths are her visual motor skills. When she is focused she is able to write her first name and majority of her last name and straight line shapes with curved shapes emerging. Emma may benefit from additional support in the classroom. She also may need heavy work breaks worked into her daily routine to assist with self regulation. Findings will be discussed with the case conference committee.
