These measures assess the health outcome of the patient whether intended or unintended as a result of care received. They evaluate if the goals of care have been achieved. Outcome measures include mortality, morbidity and health related quality life issues. Developing outcome measures comes with numerous challenges. Gathering data about a particular outcome, and attaining detailed information only found in patient medical records can be difficult and costly to obtain. Outcome measures reflect the needs that are important to patients and should be therefore be developed with patient needs and preferences in mind. It is also important to remember the impact social determinants of health and differences in patient population when developing health outcome measure. The last measure of quality improvement is patient experience measures. These measures offer feedback on the patients’ experience based on care they received. They measure not only the interpersonal aspects of care but also about information provided by physicians, to the ability of them being able to make appointments for care. Patient experience measures should be representative of the needs, values and preferences of patients. It can be understood of as a roadmap for solving problems and improving products in health care services. The defining process involves identifying and clarifying an issue. It also includes identifying goals and outlining the target process. Our next step is deciding the parameters to be measured, the methods in which they are going to be quantified, collecting the data and carrying out the measurements. Our third process is that of analysis. It involves identifying gaps between actual and goal performance. The fourth process is improvement.
