D-This writer met with the patient as she was placed on HOLD. She reported feeling alright on her dose, but denies the need for a dose increase when offered by this writer. Please note, this writer cleared the HOLD for the patient to dose first and overheard the patient discussing about her situation of being risk of being detox from the clinic and feels it is unfair as she's been a patient at CSAC for 4 years without causing any issues. This writer addressed the patient comment and made it clear that this clinic tends to not detox patient without a reason. In addition, this clinic is not purposely removing the patient from treatment; however, there's concerns as to how the patient is taking her benzo. medication and the concern factor of the patient losing 30 pills, but the patient miraculously locates the benzo. medication on her apartment floor, couch, etc. whereas the patient medication should have been in her bottle, stored, and safely placed in her home. In addition, it was reported that the patient's friend may have stolen her medication; however, the patient admits to making a comment like that, but is uncertain if the allegation is true as she haven't confronted the friend. Patient was emotional and detailed about an off and on relationship with her ex, who is also patient here at the clinic.
