During the week of the audit, many order selectors were observed performing tasks above and beyond those stated in the company’s documented preferred methods and those considered and allocated in the engineered labor standards resulting in inaccurate and understated performances achieved against standard. For example, observed were labeling of cases after placement to the pallet and traveling out of sequence to obtain base items of multiple cases. The specified methods and engineered standards require a selector to place a case label during the obtain of the case and requires a selector to travel in slot sequence as reflected on the assignment case label sheets.

The “Preferred Methods Selection Activity” document dated 8/26/2010 is obsolete and does not reflect the current engineered labor standards for selection. The most current preferred methods for selection dated 3/2/2016 require clarification.

Number 7 of the current preferred methods states time is given to label each case. This is an inaccurate statement and suggests that one can label each case as an external motion. The proper methodology reflecting how case labels are to be applied to cases is stated in number 19 and reads “For case labels, peel from backing while walking to the slot. Place the label on the case while obtaining the case from the slot.” To resolve this issue number 7 of the current preferred methods should be struck

Number 15 of the current preferred methods states “Read the pick label information and the case description.” This suggests time is engineered to allow one to read details of the case label and case whereas time is allocated to conduct only a brief eye scan of the label and case. The method as currently written should be rewritten to reflect the actual requirement in a clear and concise manner.

    1) At Peregrine, the risk factors, especially those aligned with control environment factors and the “tone at the top” are discussed. The risks included having a paper based confirmation system for the entire business; no segregation of duties, which meant that only one or very few people were working on the financial statements, and in this case Mr. Wasendorf was the only one and had altered all bank documents using only Photoshop, excel, scanners, and printers. Continuing on, time off was not required at Peregrine, which could have ultimately discovered this fraud much earlier, by allowing someone else to sit in the absentee’s seat and review all of their work. Additionally, Peregrine lacked internal auditors or an audit committee at the time. The presence of the internal auditors could have decreased odds of misstatement incentives to commit fraud by management, called fraudulent financial reporting, and overseen…
