The setting of them being by the train station, could very well foreshadow the two lives changing in time. In the story, Hemingway never mentioned that the train arrived or anything, so maybe that could foreshadow that the baby was not going to come due to the choice of her not going through with the pregnancy even though the story never says if she has the baby or not. When she says that the hills look like white elephants, it could foreshadow that the unnamed operation is an abortion because the hills look like pregnant elephants, which she could be using to describe her
