Kidney Disease
Mr Goodpasture has presented with the signs and symptoms associated with worsening of his chronic kidney disease (CKD) including signs of fluid overload and uraemia defined as the presence of excessive amounts of waste such as creatinine and urea in the blood (Mosby's Dictionary of medicine,nursing and health professionals, 2009). These include pitting oedema, basal crackles, agitation, increased temperature and premature ventricular complexes. There are also signs of electrolyte imbalances in the muscle weakness and diminished patellar reflexes. Supportive care is also known as medical management or conservative kidney management (CKM) of end stage CKD. It lets the disease run its course and focuses on treating the symptoms. (Zios.H, 2014) As stated in the booklet from the Renal Resource Centre ‘Supportive care aims to preserve kidney function for as long as possible but cannot stop the decline in kidney function. It does not replace kidney function. Dialysis and transplant whilst effective treatments do not fully replace kidney function and do not cure the underlying disease or associated illness. Choosing supportive care means that you accept that your loss of kidney function will progress and that there is a high chance that this might lead to your death. There will be increasing bouts of fatigue and tiredness as well as nausea and maybe vomiting his immune system will also be compromised as the CKD progresses no matter what therapy he decides on. This is why a multidisciplinary approach is most needed The Goodpastures need access to Social Workers, Home Help, and Counsellors maybe spiritual guidance as well as Palliative care ,nursing and Doctors. Dietetic, physiotherapist and occupational therapist (Zios.H, 2014) will need to all play a role in the Goodpastures life so as to support and care for Mr Goodpasture and his family throughout the final stages of CKD to aid in the accomplishing the best outcome possible for Mr Goodpasture no matter what decision he
