On Tuesday, December 27, 2016, at 4:00 p.m., the Investigator arrived at the Treehouse Foods Incorporated, bakery manufacturing plant at 16100 E. Foothill Boulevard, Azusa, CA 91702. The Investigator conducted an Employer Level AOE/COE investigation with the Production Manager of Operations, Mr. Gordon Potter, Lead Line Supervisor, Mr. Brigido Luna, Supervisor, Mr. Najera Santi, and Supervisor, Mr. Pedro Casa. During the interviews the investigation revealed a work-related back injury which involved Claimant Mr. Jose Puentes by Supervisors Brigido and Casas. Per Brigido, the incident was witnessed by a former employee, Mr. Benny Mendoza. The insured provided Mr. Mendoza’s contact phone number for a follow-up.
