Received call from daughter Patricia about Emma's behavior, Daughter expressed some concerns about "miscommunication" between staff and shifts. Stated she noticed for last couple of weeks toileting been very stressful for Emma. She requested to make it less anxious event by keeping her informed and reassuring. Also she expressed her thoughts of family's decision to put her "through so much" and stated she is glad the family realized not to "stress her more by more tests". Daughter stated she just wanted to give a summery of her visits and wished if there was only one person "link" who can follow-through everything as message get missed between so many staff members. Writer informed that UC is the best person to approach and is the main link between the team. Writer informed her about toileting plan and moving Emma to a supervised table to ease her anxiety also updated her on pharmacological interventions to ease anxiety, Daughter stated that personal care giver will be another link for communication and for continuity of care, daughter stated she will come in and see the social worker late this evening.
