It is the job of the circulating nurse, who is clean not sterile to observe the practices of other team member to insure sterility is maintained. It is surgical protocol that the operating room establish a sterile field and environment to prevent pathogenic contamination and to have full infection control and prevention. Sterility is established in the following manner. Scrubbed staff prior to entering the OR must conduct a wet scrub hand hygiene procedure. They then must dawn the appropriate sterile personal protective equipment to insure their own sterility as well as the working field. Either the surgeon or circulating nurse than uses a surgical prep solution such as Choraprep or Iodine to prep the surgical site before draping occurs. Sterile drapes and covers are used to surround the surgical site to ensure and establish a sterile field. All equipment used in close proximity of the sterile field must also be adequately draped. All tool and instrumentation used within the sterile field must be properly sterile and sterilized prior to use. The circulating nurse and scrub tech insures equipment sterility by meticulously check the packing in which the item is contained. At any time during the procedure if sterility is compromised the team must be notified and action must be conduct to correct this breach and prevent further contamination. Measuring patterns of outcomes, processes and practices regularly help and OR run efficiently and improve on areas of need. In the Elliot OR there are multiple white boards that outline statistical information to help the staff focus and to act as a reminder for areas of need. They as well highlight proficiency levels. There are many metrics in which an OR can assess but a few I have found the Elliot sees as a priority which include, case start time, turnover rates, case estimate duration vs. actually rap up, complications, appropriate staffing, outcomes and patient satisfaction to name a few. These are measured with the initiative to help increase OR productivity, satisfaction, and increase patient
