This essay will be identifying key elements of a chosen individualised care plan. In addition to this, this essay will also demonstrate the use of the Roper, Logan and Tierney (RLT) Activities of Living Model (Roper et al, 1996) for assessing, planning, implementing and evaluating (A.P.I.E) the care of a chosen individual. Together with this throughout the essay, whilst identifying the key elements and demonstrating the use of RLT there will be a discussion which will consist of strengths and limitations when adopting both the RLT model and the APIE framework in relation to fulfilling the biopsychosocial needs of an individual. Besides this the essay will present examples from a chosen formative care plan to highlight and back my discussion.
