I. Introduction
a. Adolescents who engage in sexual activity place themselves at risk of sexually transmitted diseases (STD) or unplanned pregnancies.
b. Many teenagers and adolescents are misled or hold fabricated beliefs concerning STD and teenage pregnancy. By every measure, adolescent sexual behavior is harmful to the well-being of all involved.
c. Each year, one in every 20 adolescents and young adults will develop a new STD (Department of Reproductive Health and Research, 2007).
d. The Centers for Disease Control and Prevention (2010) estimates that there are approximately 19 million new STDs each year and almost half of them are among young people between ages 13 and 24. STDs cost the United States health care system an estimated $15.9 billion annually (CDC, 2010).
e. According to CDC (2011), teen pregnancy accounts for nearly $11 billion

In 2011, there were 12,910 births to teens ages 15-19 in Georgia or 37.9 per 1,000 teens (OASIS, 2011). In 2009, there were 20,886 teen pregnancies in Georgia; 10,170 were among African-Americans and 2,544 were among Hispanics and there were 5,478 repeat pregnancies among teens (OASIS, 2011).
II. Problem Statement
a. The problem is teenage pregnancy and sexually transmitted diseases are epidemics that plaque this Southwest Georgia rural city.
b. Georgia is ranked eighth out of 51 states and the District of Columbia on 2010 final teen birth rates among females aged 15-19 with 1 representing the highest rate and 51 representing the lowest rate (Martin, Hamilton, Ventura, Osterman, Wilson, & Mathews, 2012).
c. On a similar scale, where 1 is the highest teen pregnancy rate and 51 is the lowest, Georgia is ranked 9 out of 51 states and the District of Columbia in pregnancies to females aged 15- 19 in 2005 (Guttmacher Institute, 2010).
d. In Georgia, the teen birth rate births per 1,000 females aged 15-19 in 2010 was 41.4% and in the United States 34.2% (Martin, Hamilton, Ventura, Osterman, Wilson, & Mathews,
