The Humanism Theory of Personality suggests that Hemingway did not receive unconditional positive regard from his parents and other figures while he was a child. His self-esteem may have been negatively affected and he has an internal conflict between the ideal self and self-image. His maladaptive thoughts and behaviors may have been a result of not feeling as though he were a worthwhile human being. The Diathesis-Stress Model is the most likely cause of Hemingway's disorder(s) because it takes into account both cognitive and biological factors. Hemingway grew up in a very stressful environment that continued throughout his adulthood. Mr. Hemingway suffered emotional and physical abuse as a child, he was also involved in three wars, multiple head injuries, close family members committed suicide (father, sister, and brother), and his family has a history of mental illness. His genetic predisposition, triggered by his trauma, caused his distorted perceptions and emotional imbalance. Upon 72 hours he should be evaluated for a WIC-5250 7 day involuntary psychiatric observation and treatment. His alcoholism treatment should begin with detoxification with an observation for withdrawals. He should also receive individual counseling and groups for relapse prevention and emotional support. He should receive inpatient group therapy for his Bipolar Disorder including symptom management, medication management, and relapse prevention. Hemingway should also be seen by the inpatient psychiatrist to review his medications for the Bipolar Disorder. Appropriate blood tests and urinalysis should be performed to check for substance abuse, medication levels and kidney and liver function. Due to the complicated diagnosis of Bipolar Disorder, PTSD, alcohol abuse, and Borderline Personality Disorder, Dialectical Behavioral Therapy is likely the best therapy to support the combination of treatment issues. He would benefit from both groups and individual therapy.
