EE Case Summary

Decent Essays
EE stated that he is currently suffering from dizziness, nausea, fatigue, hot and cold sweats and stomach cramps. EE stated that the symptoms began on 07/30/2017. EE stated that at first he was feeling nausea and slightly fatigue. As the week went on he began to experience hot and cold sweats and stomach cramps. On 08/03/2017 EE schedule an appointment with Dr. Melissa R. Moore (Northside Medical Professional-1605 Nashville Hwy Ste 200, Columbia, TN 38401) to discuss treatment options. While at the doctor office EE received a steroid shot to help subdue the effects of his symptoms. EE does not have a follow up appointment. EE does not have RTW date as of yet. EE has nothing further at this time.
