In the history, I would like to know past hospitalization, pelvic or prostate surgeries, radiation, chemotherapy, cancer, trauma, MI, stroke, allergy to medications or food, route, frequency and quantity of Zyrtec (OTC), if Erectile Dysfunction (ED) is getting worst or stable and if he had tried any medication for ED. Also, medication history that includes use of herbs, supplement and caffeine. Social history that includes smoking, alcohol abuse and use of illicit drug. Moreover, diet, physical activity, relationship issues, stressors at work and home. Psychological factors that includes depression, mood disorder, anxiety, insomnia. Furthermore, I would like to know if patient has any fatigue, genital pain or altered shape, neurological disorder, peyronie’s disease, benign prostatic hyperplasia (BPH) and hypogonadism. Sexual history that includes situation, desire, orgasm, ejaculation, monogamous or heterogamous relationship, nocturnal or morning erections, frequency of sexual activity and planned or spontaneous. I would like to know lab values of thyroid stimulating hormone (TSH), prolactin level, follicle stimulating hormone (FSH), complete blood count (CBC), testosterone level, and urinalysis for the presence
