SC placed call to Dynamic Home Health Care on 2/5/2016 to inquire about the last time a nurse went out and saw the Pa to review care and/or medication. Angela answered the phone and the SC identified self and stated reasons for calling the call was transferred to Lynda. SC again stated reason for calling and Lynda placed SC hold for several minutes while she finds the information the SC has request. When Lynda returned to the call she stated that the last visit was approximately 5 months ago but there were not notes detailing what occurred at the visit and she is out on leave. The SC asked Lynda how often does the nurse go and she stated around every few months. The asked if the nurse provides any medication oversight and she said she was not sure. That which may not be meeting most of the Pa’s needs. She then stated that the family will go this weekend to make sure she is ok. The SC informed Margarete that the Pa has not been talking her medicine as prescribed that that poses a major problem, because the lack of medication adherence could be contributing to the Pa’s declining health and impairing her functions to performed ADLS and ambulates with device. The SC informed Margarete that the SC will create a new care plan to accommodate the Pa’s new unmet needs. Margarete asked if the Marshelda her gdtr will still be the DCW and the SC informed her that the Pa’s determines that. The SC asked for the DCW contact information and Margarete provided the DCW full name and phone number. The SC thanked Margarete for her time and information and end call.
