Dronaderone is the analogue of amiodarone which is used for patients with persistent or paroxysmal atrial fibrillation. It is used for the maintenance of sinus rhythm and persistent or paroxysmal atrial flutter who are in sinus rhythm or undergoing cardioversion after initiation of the drug. Dronaderone (Multaq) is sodium and potassium ion channels blocker, like amiodarone. It is also an alpha and beta adrenergic receptor antagonist. Dronaderone is a benzo furan derivative like amiodarone, with the addition of methyl sulfonamide group which reduces the lipophilicity. Lipophilicity reduces the drug to cross blood brain barrier, and this has shown in less adverse events in comparison to amiodarone. Dronaderone is not used for the patients who has heart failure or left ventricular hypertrophy.

On the other hand, dronaderone is the alternative option in patients who have develop adverse effects from amiodarone or has the underlying disease conditions. A clinical trial named DIONYSOS (Efficacy and safety of Dronaderone verses Amiodarone for the maintenance of Sinus Rhythm in patients with Atrial Fibrillation) compared dronaderone 400 mg twice daily verses amiodarone 600 mg/day for 28 days. The results showed that dronaderone was less effective in maintaining sinus rhythm. Rate of recurrent atrial fibrillation was 63% in dronaderone group and 42% in amiodarone group. Dronaderone was associated with adverse effects such as bradycardia, QTc prolongation, and cutaneous rash.
