Review of the medical record indicates that he was admitted to MMH on 9/3/16 for hip pain. He had a CT with stone protocol, which showed bony abnormalities which are suspected to be metastatic to bone and liver. Dr Swanson for oncology was consulted. His other medical history includes AAA repair, aortic stenosis, osteoarthritis, COPD, AVR and Lyme disease. At today’s visit he is accompanied by his daughter. He is awake, alert and oriented. He reports that he was told in the hospital about his diagnosis, but was not told about his prognosis. He is awaiting a call back from Dr Swanson office to schedule a follow up appointment.
