The hospital verified this insurance through the Passport system and then submitted its initial a claim to Medicare on June 11, 2014. The claim was rejected, and then the hospital learned that the patient was insured through SelectCare. RF is a 49 year old man who came to the shelter off the streets. He had a history of hospitalization and was diagnosed with Bipolar Disorder and Anxiety Depression. With the help of case management, RF was able to maintain his mental health treatment and sobriety. He started off by seeing the MHAMC (Mental Health Association of Morris County) psychiatrist for medication monitoring and counseling. 8/19/2015: The social worker was asked by the nurse to assist a patient, per Dr. R. Sundaram. The patient was requesting a letter to state his competence in supervision of his two children. The Social Worker is not qualified to address a letter such as this and consulted her supervisor about this matter. The Clinical Services Officer, Nursing Service Director, LPN – Team Lead, Dr. R.
