Circumstance: Ms. Smalls (MHP) made phone contact with Ms. Canady (MHS) regarding Donovan.
Action: MHP and MHS discuss the ingoing issue with the school bus picking and dropping Donovan off to school. MHS expresses concerns of Donovan not getting to school on time. MHS express speaking with the bus driver and Donovan’s teacher about the issue. MHP explain discussing the issue with the DSS worker, however the DSS worker has not follow-up with additional information. MHP will try to reach the DSS worker for additional information. MHS reported Donovan is having problems in school and the teacher is asking about medication. MHP explain Donovan’s mother took the youth off medication. MHP explain Donovan’s mother has rights and DSS would need to
