This is in response to your correspondance to me, dated 02/09/16, regarding Disciplinary case 12-L25-0795.

Per Department Order 803 INMATE DISCIPLINARY PROCEDURE
803.09 REVIEW AND APPEAL
1.2.4 First Level Appeals of Class A and B Violations - Inmates may submit an appeal of a Class A and B violation within five calendar days of receiving the guilty findings using an appeal of Disciplinary Charge form or an Appeal form designated for that purpose.

1.2.5 Second Level Appeals to the Director - An inmate who rejects the response may submit a second level appeal within five calendar days of receiving the Step One decision, using the Appeal of Disciplinary Charge form, through the Disciplinary Coordinator’s Office, who shall forward the packet
