In addition to the survey, one student submitted the Disability Support Services Accommodations Agreement form from Disability Support Services. Another said he qualified for accommodations, but had not requested the notification paperwork for the Winter Semester. He wanted to wait to see if he would needed to use it. After discussing the formats of my tests and assignments, both students said they felt that they could successfully complete my course without help from test services (F. Capese and F. Robins, personal communication, January 12, 2016).
