Describe specific examples of where you prioritized your actions or assessments based on the needs of a patient or a group of patients that you cared for. Provide details on your highest priority action or assessment. Why did you decide one action or assessment was more important than another? Describe a situation where you had to revise a plan of care for a patient based on changing circumstances. Describe a period of care when you were especially efficient in completing patient care in a short period of time (e.g. you brought supplies into the room to do multiple tasks at once.)
While performing an assessment on a vaginal primipara, I noticed that she started crying. I started to assess the reasons why. She reported that she was exhausted and had not slept in four nights, her nipples hurt and breastfeeding was not going as planned. Her newborns bilirubin levels were a little on the high side and needed formula supplementation to speed up excretion. I explained to her that her feeling were completely valid, that she had planned and pictured a beautiful experience with her newborn and reality was not meeting her expectations. I asked what she felt she needed at the time and her response was sleep. i looked down at my brain and saw that if I combined several tasks I could provide a four hour window of no disturbances. Describe a situation of role modeling for healthcare members, family members, team members, other student nurses. Describe a time that you initiated communication with a physician. This week I read an article stating that research shows that starting a general diet earlier helps kick start GI motility. The article was brought to my attention by a resident. After reading the article and looking at the evidence, I learned that early implementation of a general diet (within a couple of hours) does not compromise safety or comfort. Early feedings in postcaesarean women coralign with increased patient satisfaction and decreased perception of pain. The new research was implemented by half of the nurse on the unit by the next day. I was shocked how fast the news had spread and
