The Via Christi social work department creates a care discharge plan, considering the patient’s strengths, the patient’s current level of care, and patient current resources, such as available caregivers, if appropriate. Discharge plans can include, but not limited to, a referral to a home health care agency, a direct-admit referral to a long-term facility, a direct-admit to a rehabilitation facility, or help obtaining discharge prescription medications.
