The two reports and forms that I chose were encounter forms and discharge summary forms.
The difference between the two are:
1. Discharge summary forms are what they say. They are commonly found within hospitals. When a patient is discharged from a hospital, a doctor provides them with a discharge summary of what they were seen for following instructions on what type of care is needed after they are released from the hospitals care.
2. The next form is an encounter form. Encounter forms are documents or records used to collect data about given elements of a patient visit to a physician’s office. It’s a very important form in the medical billing and coding department because it has codes to be able to bill you accurately.
