CM-Toby completing discharge ISP for Julia. CM was unable to conduct a formal discharge meeting due to no contact with the Saginario family. Prior CM conducted a CFT back on 5/11/17 and since then CM-Toby have not been able to get in contact with the family since that date. CM-Toby made multiple attempts to call the family, left voice messages and no contact letters was even sent out to the Saginario family. One of the most important aspects of discharge planning is making the arrangements for continuity of care, which can include follow-up appointments, referrals to out-patient therapists and services, medication plans and prescriptions, and other arrangements. A good discharge planning may be one strategy to protect against early readmission for psychiatric patients because it provides stability of the patient’s medical condition, and prepares the patients emotionally and practically cope in the community, with appropriate aftercare supports. Patients with a history of repeated admission are most at risk and require special efforts to break the readmission cycle.
