ALLEGATION
Tommy (15) is a diabetic and insulin dependent. R/s it was reported that Tommy ran away last night and he was found on Interstate I-77. R/s reportedly Tommy was in a facility for 4-5 days for trying to kill himself. R/s Angela (mom) is very neglectful, she is more concern with her own life than with the children. R/s Angela is not getting up to wake the children for school. R/s Tommy is without his medication. R/s sometimes Tommy and James (14) are hungry. R/s Tommy is an intermittent homebound student.

R/s James reported Tommy is now out of the facility. R/s she called Tommy (father) and he did confirmed what James had reported. R/s the father stated that Tommy was in the facility for 4 or 5 days, but the school thought Tommy was in the facility for two weeks. R/s several attempts had been made to reach mom regarding child’s school work. R/s the children come to school hungry. R/s they are doing state testing this week at school.

CLIENT CHARACTERISTICS
Tommy Lewis Bennett is a white male, born on 01/30/2001. He is a diabetic and insulin dependent. He is in the 8th grade Oakridge Middle School.
